To me the three flare insignia looks like an ordinary RAdm pin where one of the flares broke off or wasn't attached to the base.
Regarding the second image: cool to see all the admiralty braids together, however odd that there is no great difference between RAdm and VAdm exept the placements of those black/sivler bands -- though odd that for RAdm it stops on the base braid and for VAdm it goes above (and around?) the top and bottom gold trim.
